We are changing how we organise chronic disease reviews.  From now on, please book all your chronic disease reviews in the month of your birthday.  

While we change to this new system, for most patients this will mean that the chronic disease reviews this year are earlier or later than expected.  

If you have already had your annual review for a chronic condition this year (since 1st April) you will not need to have another.
